[...] The ignorance, rarity, and regional disparity in gerontological orientation and coordination structures may be involved in the polymedication phenomenon. Finally, ignorance of non-medical and preventive approaches leads professionals to over-prescribe. The major risk of polymedication is the iatrogenic risk, which is the appearance of health problems and diseases caused by prescribed medications. Medicinal iatrogenies can be the origin of new prescriptions. To reduce polymedication and prevent the iatrogenic risk, it is essential to rely on the complementarity and synergy of different preventive and curative therapies, with medication being just one of the elements of the panel. [...] Question 3 : The analysis of prescriptions in the elderly makes a quasi-systematic polymedication. This polymedication is linked to the important frequency of polypathologies in the elderly due to difficulties in hierarchizing prescriptions that refer to a difficult hierarchization of accumulated diseases. This leads to a superposition of prescriptions. Polymedication can therefore be linked to the number and variety of interveners and prescribers but also to the insufficiency of training in geriatrics of healthcare professionals leading to ignorance of the undesirable effects of prescribed medications. This gerontological team, whose line of conduct is guided by communication and organization, facilitates the intervention of trained professionals. In fact, an effective gerontological team adopts an organization based on consensus regarding, among other things, care objectives and decision-making. Collective reflections on a difficult situation can benefit other patients and have reassuring effects for the members of the gerontological team, thus drawing on the group effect to manage a situation; which reduces the level of stress of the professional facing a complex situation, and therefore optimizes their reasoning. [...] Question 2 : A paradigmatic change leads the elderly to favor home maintenance and delay institutionalization. The healthcare system adapts. This concept of 'home maintenance' considers the entire set of means implemented so that an elderly person can remain at their home, in the best conditions.
